Patient Requirements

To be seen at the Matthews Free Medical Clinic, you must:

1. Live in Mecklenburg or Union County
2. Be 6-64 years old
3. Not have Medicaid, Medicare, VA or private insurance
4. Attend an eligibility screening. Please call (704) 841-8882 to schedule a new patient screening
   

Recertification

• MFMC recertifies patients every 12 months from their starting certification date.
• You must bring your current year's tax return, most recent payroll stub, recent utility bill, and disability statement, if applicable.
• To review and download the forms you need to bring with you to your appointment, please click here.

What to bring with you

1. Picture ID
2. Social Security card, if available
3. Two (2) consecutive Payroll stubs (most recent) or letter from employer
4. Utility bills with your name on it
5. Mortgage statement
6. Two (2) consecutive bank statements
7. Current year's tax return
8. Disability statement, if applicable
9. Food stamp letter, if applicable
10. Unemployment benefits letter, if applicable
11. Child Support, if applicable
